# Intuitive Surgical Stock Before Q3 2026 Earnings: Boston Scientific Shows How Rivals Take Share, and Where to Look — InvestMoat Research

_Intuitive reports on October 20 with Medtronic's Hugo and J&J's Ottava both cleared in the US, and I think Boston Scientific's lost PFA share shows the rivals' clearance date matters less than how often a customer can try them._

## Summary

Intuitive Surgical closed at $426.70 on October 16, 2026, ahead of third-quarter results on October 20. Its July slowdown came from GLP-1 drugs cutting bariatric surgery, ACA subsidy changes and deferred benign cases, not from rivals, because Medtronic's Hugo was cleared only for urology and J&J's Ottava wasn't cleared until July 22. Boston Scientific shows how share moves in medtech: its rivals in pulsed field ablation were cleared at about the same time it was, and US share only shifted later, in a market where labs can try a rival's catheter case by case. A da Vinci is chosen once per capital cycle, so I argue the place to watch for the same shift at Intuitive is the trade-in, when a hospital replacing an old system could buy a different robot, rather than procedure growth.

## What would prove this wrong

Before Intuitive reports its second quarter of 2028, either its trade-in transactions (mostly US) fall year over year for two quarters in a row while US da Vinci procedure growth stays at 10% or above and Intuitive cites competitive evaluations, longer sales cycles or a rival's clearance as a reason, or Intuitive names a competitor as a reason for lost US placements. Either would mean hospitals are choosing rivals at the capital decision, and that the trained-surgeon moat I lean on here slows share loss less than I think.

ISRG price: (live) · Close on October 16: $426.70 · US da Vinci procedure growth: 12% · Q3 2026 results: Oct 20

Intuitive Surgical reports its third quarter on Tuesday, October 20, after the close. At the October 16 close the stock had recovered a good part of what it lost after the July report, when it fell more than 11% the next morning on a beat because US da Vinci growth had slowed, and it was still about 30% under its January 2025 peak. I'm writing this before the print so the claims below can be checked against it.

Since July the stock has been argued about in terms of the rivals. TIKR put the summer drop down partly to Ottava, which it called the first credible challenger to da Vinci in two decades. On August 12 Oppenheimer upgraded the stock with a $500 target after field checks found workflow problems with Ottava and a large footprint with Hugo, and it concluded that US competition isn't a factor in Intuitive's growth. I think the rivals had nothing to do with the slowdown so far. But one of the other medtech leaders in our coverage has just lost US share to Medtronic and J&J, and how that happened says where to look at Intuitive.

## How Boston Scientific lost share in pulsed field ablation

[BSX](/stocks/bsx) didn't lose its lead because a rival got cleared. Medtronic's PulseSelect was the first pulsed field ablation system the FDA approved, in December 2023, and Boston Scientific's Farapulse followed seven weeks later. Farapulse still became the leader, which Mahoney says it remains. The share shift came later, after Medtronic's Affera system and Sphere-9 catheter were approved in October 2024, a single catheter that maps the heart and ablates with either pulsed field or radiofrequency energy.

**Pulsed field ablation in the US, from the first approval to Boston Scientific's second guidance cut. Dates and figures as reported by the companies, the FDA and trade press on the date shown.**

By July 2026, about 21 months after Sphere-9 was approved and two and a half years after the first rival was, Boston Scientific's US electrophysiology growth was down to 3% while Medtronic's was around 40%, in a quarter that had an extra week. I think what moved it was a rival product that labs wanted to try, and Sphere-9's timing fits, though Mahoney named no rival and J&J and Abbott were selling PFA systems too. What I'm surer of is how easy it was for labs to try one. An ablation catheter is chosen for each case. An electrophysiologist can use a Sphere-9 one week and a Farapulse the next, and the lab's decision is the sum of a lot of small ones. That's the part I think doesn't carry over to Intuitive, and it's why I don't think procedure growth is where a da Vinci share loss would show up first.

**The pillars that protect a device maker's revenue, for the two medtech leaders in coverage that now face Medtronic and J&J. Statuses are read live from each stock's analysis.**

| Ticker | Regulatory Lock-In | Learned Interfaces | Network Effects | Transaction Embedding | Scale Economics |
| --- | --- | --- | --- | --- | --- |
| **A robot bought once per capital cycle** | | | | | |
| [ISRG](https://www.investmoat.com/stocks/isrg) | strong | strong | intact | intact | intact |
| **A catheter chosen case by case** | | | | | |
| [BSX](https://www.investmoat.com/stocks/bsx) | strong | intact | weakened | intact | intact |

Both companies have regulatory lock-in rated strong, and Boston Scientific's case shows the limit of that rating. It describes the lag before a rival gets its own clearance, and in PFA there wasn't one, because the rivals were cleared alongside Farapulse. At Intuitive the lag is closing procedure by procedure. Hugo is cleared in urology and Medtronic filed in June for general surgery, including hernia, and gynecology. Ottava is cleared in ten general surgery procedures. The framework keeps Intuitive's rating strong and marks it as narrowing, with the narrowing ending only if those filings aren't cleared by the end of 2027. I'd go further and treat the lag as already gone wherever a rival is cleared, though that's my reading rather than the stock page's.

Two pillars differ between the two names. Learned interfaces is rated strong at Intuitive and intact at Boston Scientific, and the Boston Scientific note says retraining an electrophysiologist on a new catheter is measured in cases, enough to slow switching but not to prevent it. Network effects is rated weakened at Boston Scientific, with a note citing the US PFA share loss as proof that labs will switch, and intact at Intuitive, where surgeons who move hospitals pull da Vinci purchases with them. Those are both ratings of the same thing, the trained clinician, and the bigger difference behind them is how often the customer decides. A da Vinci is a capital purchase and its instruments only fit the console, so a hospital makes the switching decision once per system, when it replaces one or adds one.

> **The test I'd apply** — When a medtech leader's rivals are cleared, I'd look for the moment the customer actually chooses. For a catheter that's every case, so share can move within a few quarters of a rival product labs want to try. For a surgical robot it's the trade-in or a new system, so the number to watch is how many hospitals replacing an old da Vinci buy another one.

## What management said slowed in the second quarter

**Intuitive's second quarter of 2026, from the earnings release and the call. Figures as reported on July 16, 2026.**

| Measure | Q2 2026 | Compared with |

| Worldwide da Vinci procedure growth | About 15% | FY2026 guide of 13.5% to 15.5%, nearer the midpoint |
| US bariatric da Vinci cases | Down high single digits | Attributed by the CFO to rising GLP-1 use |
| ACA premium subsidy expiry | A modest adverse impact on US procedure growth | Based on customer feedback; no precise estimate of exposure |
| da Vinci systems placed | 468, of which 246 da Vinci 5 | 395 and 180 a year earlier |
| Trade-in transactions | 144, mostly US customers upgrading to da Vinci 5; about half of US placements were trades | 83 a year earlier |
| Installed base | 11,710 da Vinci systems | 10,488 a year earlier |
| US da Vinci utilization per system | Up 3% | Described by the CFO as a healthy level |

None of the reasons management gave was a rival. The CFO named bariatric cases falling as more patients take GLP-1 drugs, a modest hit from patients losing enhanced ACA premium subsidies, and slightly slower growth in benign procedures, some of which can be put off. He also allowed that a business this size is running into the law of large numbers, and nobody on the call asked about Hugo or Ottava. Hugo was cleared only for urology, and Ottava wasn't cleared until three weeks after the quarter ended. I don't see a way for either robot to have moved a quarter of US procedure growth.

That's why [LLY](/stocks/lly) and [NVO](/stocks/nvo) are in the scorecard below. They aren't part of the moat comparison. They're the demand side, the two drug makers whose GLP-1s are taking volume out of bariatric surgery, and they stand for the part of Tuesday's number that no moat in the framework protects against. A weak US procedure figure would mostly be about them and about insurance coverage. A strong one wouldn't clear the rivals either, because neither has had a full quarter of general surgery in the US.

## What Hugo and Ottava are cleared to do

**The US-cleared rivals this article weighs, as described by each company and in reporting at the time. Dates as shown.**

| System | US clearance | What's cleared | What's known since |
| --- | --- | --- | --- |
| Medtronic Hugo | December 3, 2025 | Urologic procedures including prostatectomy, nephrectomy and cystectomy, about 230,000 US surgeries a year by Medtronic's count | First US commercial case at Cleveland Clinic by February 2026. 510(k)s filed on June 3, 2026 for general surgery, including hernia repair, and for gynecology. Oppenheimer found it takes more operating room space than da Vinci |
| J&J Ottava | July 22, 2026 (De Novo) | Ten general surgery procedures, including cholecystectomy, gastric bypass, gastric sleeve and hiatal hernia repair. An inguinal hernia trial is underway | Launching with select US customers. Four arms built into the operating table. J&J's Q3 release gave no case or placement counts. Oppenheimer found workflow problems with sterility and setup. TIKR called it the first credible challenger to da Vinci in two decades |
| CMR Surgical Versius | October 14, 2024 (De Novo) | Cholecystectomy in adults | A smaller, privately held rival; no US placement figures disclosed |
| Distalmotion Dexter | Q4 2024 (De Novo) and May 2025 (510(k)) | Laparoscopic inguinal hernia repair and cholecystectomy in adults | A smaller, privately held rival; no US placement figures disclosed |

The overlap with Intuitive's growth is narrower than the list of ten makes it look. Management named general surgery as the main US driver for 2026, and cholecystectomy and hernia repair as the high-volume procedures it's going after. Cholecystectomy is the one real overlap today. The big hernia procedures are inguinal and ventral, and Ottava is cleared only for hiatal hernia, with inguinal still in a trial. Hugo's general surgery filing, which includes hernia, is the clearance that would widen the overlap, and it could come at any time. Two of Ottava's cleared procedures, gastric bypass and sleeve, are bariatric, the category GLP-1s are shrinking.

## Where a share shift would show up first

In the second quarter Intuitive recorded 144 trade-in transactions, up from 83 a year earlier, mostly US customers upgrading to da Vinci 5, and about half of all US placements were trades. Each was a hospital at the point where it could have bought something else. In the second quarter that wasn't much of a test. A large hospital's general surgery program had no large-cap alternative. The small, privately held rivals cleared in the US, CMR's Versius and Distalmotion's Dexter, cover cholecystectomy and, for Dexter, inguinal hernia, and Hugo was cleared only for urology. From the third quarter on, with Ottava cleared and Hugo's general surgery filing pending, it starts to be one. If hospitals with a cleared alternative keep trading up to da Vinci 5, the surgeon lock is doing what the rating says. If trade-ins fall while procedures hold up, I'd read it as hospitals pausing to compare.

**Trade-ins rose with the da Vinci 5 upgrade cycle and have run at 119 to 146 a quarter since the third quarter of 2025. Hugo was cleared for urology in December 2025 and Ottava for general surgery three weeks after the second quarter ended, so Q3 2026 is the first quarter with a large-cap rival cleared in US general surgery. A fall from this range while procedures hold up is the signal I am watching. The Q1 2026 dip came before any large-cap clearance in general surgery, which gives a sense of the ordinary noise.**

## Where I could be wrong

The objection a holder would raise first is that trade-ins follow the da Vinci 5 upgrade cycle, whatever the rivals do. The CFO said on the July call that the upgrade cycle is progressive and occurs over multiples of years, so trade-ins could level off or fall simply because the hospitals most eager to upgrade already have. If that happens, my test would read a maturing cycle as share loss. The way I'd tell them apart is the timing and what management says. A cycle should fade gradually while a large base of older systems is still in the field. A drop that arrives as Hugo's general surgery clearance lands, or alongside comments about longer sales cycles or competitive evaluations, would be the rivals. The procedure condition in the falsifiable claim is there for the same reason, so that weak demand alone doesn't trip it.

The strongest case against the thesis itself is that I'm leaning on the clinician, and Boston Scientific's history says not to. Electrophysiologists are highly trained specialists too, and Farapulse had the lead and the installed workflows when Sphere-9 arrived. Medtronic and J&J aren't start-ups. J&J's Ethicon business already sells staplers and energy devices into operating rooms, and a hospital that wants a second robot vendor for pricing leverage has a credible one for the first time. Ottava's table-integrated design is pitched at 30% to 50% less floor space, which matters in older operating rooms, and Needham expects J&J and Medtronic to take moderate US share. If a second vendor gets in through hospitals that don't yet own a da Vinci, or through an ambulatory surgery center, trade-ins would never show it, and my test would miss the share loss.

There's also a case that I'm watching the wrong rival. Oppenheimer sees the real competition in China, from domestic systems such as Medbot, Kangduo and Toumai, and management described increased domestic robotic competition and pricing pressure in China on the July call. That's a share fight already happening, in a market where Intuitive's placements are muted. I've left it out of the falsifiable claim because it doesn't tell us whether the surgeon lock holds in the US, but a reader who owns the stock for international growth should weight it more than I have.

The bullish version is that my test is too cautious. Oppenheimer's case rested partly on Intuitive's per-click model, which opens up lower-acuity ambulatory centers, and on customized price and volume contracts that make it hard for a rival to show a hospital a better return. US utilization per system still grew 3% in the second quarter. If the rivals never get past a few showcase hospitals, the trade-in line will look fine for years and the only thing that mattered for the stock was demand. In that case Tuesday is about GLP-1s and ACA coverage, and the competitive question can wait.

The ISRG scenario ladder in the scorecard sets out what the price has to believe. Its base case assumes procedure growth of 13% to 15% through 2027 and a steady upgrade cycle. That needs the demand side to hold and the rivals to stay small. I think the second is likelier than the first, for the reason in the matrix, but I wouldn't call either settled before Ottava has had a few quarters in US operating rooms.

## What I'll check on October 20

**What I'm checking, written down before the third-quarter results. Prior-quarter figures as reported by Intuitive on July 16, 2026.**

| What I'll look at | Last quarter | What it would tell me |
| --- | --- | --- |
| Trade-in transactions and the share of US placements that were trades | 144 trades; about half of US placements | Whether hospitals replacing a da Vinci still buy another one now that a rival is cleared |
| US da Vinci procedure growth | 12%, after 14% in Q1 | How much of the slowdown is demand, which no moat fixes |
| US bariatric cases | Down high single digits | How fast GLP-1s are taking volume out of a category Ottava is cleared in |
| Any mention of Hugo, Ottava or competitive evaluations in the US | None on the July call | Whether management sees the rivals in its own pipeline yet |
| The FY2026 procedure guide | 13.5% to 15.5%, nearer the midpoint | Whether demand is still within the range the base case assumes |

> **What would break this** — In order of how likely I think it is. Trade-ins fall while procedures hold up, which is where I'd expect a share shift to show first. Intuitive names a rival as a reason for lost US placements, which would mean the surgeon lock is weaker than its rating. J&J or Medtronic start disclosing US general surgery placements at a pace that implies a real second vendor within two years, which would mean the Boston Scientific timeline applies to robots after all.

I think Intuitive's regulatory lead is already gone wherever a rival is cleared, and Boston Scientific shows that a medtech lead can outlast the rivals' clearance by years and still give way once a rival product is good enough to try. What's left at Intuitive is the surgeon, the operating room team and the instruments that only fit the console, and I think those hold better than a catheter habit did. Tuesday will say more about demand than about any of that. The trade-in line over the next few quarters is where I'll find out whether I'm right.

## Revisions

- **October 20, 2026** — Q3 2026 check, the one this article set up. Intuitive didn't give a trade-in count this quarter, but the CFO said net placements, the ones that add to the installed base, grew 40% on 546 total placements. Against last year's 427 placements and 141 trade-ins, that works out to roughly 145 trades, about level with the 141 a year ago and the 144 in Q2, in the first quarter with Ottava cleared. US net placements grew 41%, and management didn't name a rival as a reason for any lost placement. So the share-shift signal I was watching didn't show up. The slowdown did continue: US da Vinci procedure growth was 10%, after 12% in Q2, which the CFO said matched the July outlook, and full-year procedure guidance narrowed to 14% to 15%. Intuitive also took a $450 million charge to settle the US customer class action over third-party instrument remanufacturing and servicing, with injunctive relief still subject to court approval. The falsifiable claim stays holding. The shares fell about 7% after hours.
